Serious effects are rare but can include cardiac arrest and heart attacks. Oral or intramuscular forms haven't shown reported adverse effects in studies, though thorough safety testing hasn't been completed.
Longjack extract is generally well tolerated short-term in studies, but long-term safety and product quality aren't certain. It may raise testosterone levels; if levels go beyond normal, you could develop acne, insulin resistance (trouble managing blood sugar), and liver damage.
The safety data advises against use during pregnancy and breastfeeding.

Peak SX-7 by MuscleTech contains two active ingredients—PEAK ATP (adenosine) and Longjack extract (Eurycoma longifolia)—that interact with medications.
The most serious concern is PEAK ATP with dipyridamole (Persantine), a heart medication: dipyridamole can amplify both the therapeutic and toxic effects of adenosine, potentially causing dizziness, a slow heart rate (bradycardia), and fainting. If you take dipyridamole and need a cardiac stress test using adenosine, your doctor will need to stop the dipyridamole several days beforehand.

PEAK ATP also carries a moderate risk with carbamazepine (Tegretol), an anti-seizure medication—both can cause abnormal heart rhythms (heart block), and taking them together might add up to a riskier effect. Additionally, caffeine, theophylline, and aminophylline (methylxanthines—common in some asthma and breathing medications, and present in coffee and tea) can block adenosine's effects; you'd want to avoid these for 24 hours before any adenosine-based cardiac test.
Longjack extract carries a moderate interaction with propranolol (Inderal), a beta-blocker blood pressure medication—it may reduce propranolol levels and weaken its effect. It also theoretically interacts with several enzyme-processed drugs (CYP1A2, CYP2C19, and CYP2A6 substrates) and testosterone; these are minor and mostly from lab studies, not yet proven in people.

Longjack extract
Propranolol (Inderal)
Eurycoma longifolia can reduce the levels and clinical effects of propranolol.
A small clinical study in healthy persons shows that taking a single dose of a water-based Eurycoma longifolia extract 200 mg, in combination with a single dose of propranolol 80 mg, reduces the propranolol area under the curve (AUC) by 29%, reduces the peak concentration by 42%, and increases time to peak concentration by 86% when compared with control. Since the elimination half-life of propranolol did not change, it seems that Eurycoma longifolia alters the kinetics of propranolol by decreasing its absorption in the gut, and not by altering its metabolism. It is not known if separating administration will prevent this interaction.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.
Cytochrome P450 2A6 (Cyp2A6) Substrates
Theoretically, Eurycoma longifolia might increase levels of CYP2A6 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2A6 enzymes. This effect has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.
Testosterone
Theoretically, Eurycoma longifolia may further increase levels of testosterone.
A clinical study in aging males with testosterone levels below 300 ng/dL shows that taking a specific water extract of Eurycoma longifolia roots (Physta; Biotropics Malaysia) 100-200 mg daily with breakfast for 12 weeks increases total testosterone levels by 8% to 11% when compared with placebo. It is unclear whether this increase would occur in individuals with normal testosterone levels.
PEAK ATP
Dipyridamole (Persantine)
Dipyridamole can increase the therapeutic and toxic effects of adenosine.
Dipyridamole decreases the metabolism of adenosine. Intravenous infusion of adenosine in patients who are taking dipyridamole can cause dizziness, bradycardia, and syncope. Dipyridamole should be discontinued for several days prior to a cardiac stress test using adenosine.
Carbamazepine (Tegretol)
Carbamazepine might increase the risk of heart block when used concomitantly with adenosine.
Carbamazepine and adenosine can both cause heart block. Giving them concurrently might produce an additive effect.
Methylxanthines
Methylxanthines are competitive antagonists of adenosine and can block its pharmacologic effects.
The methylxanthines, aminophylline, caffeine, and theophylline, can block the effects of adenosine by acting as competitive antagonists at adenosine cell surface receptors. It is recommended that methylxanthines be avoided for 24 hours prior to cardiac stress tests.
