These probiotics are generally well tolerated in healthy adults and children. That said, live-organism products come with rare but real risks: in unusual cases, these bacteria have caused infection (bacteremia, sepsis, and in very rare instances more serious conditions) — mostly in people who are critically ill, premature infants, or have weakened immune systems.

On a day-to-day basis, some people report mild digestive side effects like bloating, flatulence, abdominal discomfort, or diarrhea, but these are uncommon. Rash and itching have been reported in one trial, though it's unclear which ingredient (if any) caused them.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
