L-glutamine is rated Effective for sickle cell disease. It's rated Possibly Effective for HIV/AIDS-related wasting, postoperative recovery, and critical illness from trauma.
The evidence supporting those uses is still being built, so results may vary.
How safe is it?
Well-documented data
Orally, L-glutamine is generally well tolerated in healthy adults. The most common side effects are gastrointestinal — belching, bloating, flatulence, constipation, diarrhea, nausea, vomiting, and stomach pain — especially at higher doses (10-30 grams daily).
Headache and musculoskeletal pain have also been reported. A few rare cases of dizziness, mania, and hypomania have been noted, mostly in people with bipolar disorder.
People with kidney or liver disease should take glutamine only under medical supervision. There isn't enough safety data to recommend supplemental doses during breastfeeding without your doctor's input.
Pregnancy appears likely safe based on available data, but talk with your healthcare provider before starting it.

Glutamine is produced in the muscles and is the most abundant amino acid in the body. It is important for many bodily functions including protein synthesis and immune function. The body is likely to go into a catabolic state if more glutamine is used than is produced, thus making it crucial for the supplement regimen of any high intensity sport.

Directions: Take one (1) scoop two to three times daily. Optimal times are upon awakening, pre and post workout. Mix with your choice of beverage.
