How safe is it?
Well-documented data
Papain is generally tolerated as a food but may irritate the digestive tract and cause allergic reactions in sensitive people. Concentrated papain and unripe papaya products are not advised in pregnancy, and there isn't enough safety data to recommend papain supplements while breastfeeding.
Bromelain is generally well tolerated short-term and may cause diarrhea, gas, or stomach upset; it should be avoided in pregnancy and while breastfeeding. Rutin is generally well tolerated and may cause headache, flushing, or rash in some people; pregnancy use is likely safe, but avoid supplement doses while breastfeeding.
Myrrh may cause diarrhea and is traditionally linked to uterine stimulation — avoid it in pregnancy. Chymotrypsin and trypsin are generally tolerated short-term, though chymotrypsin can rarely cause severe allergic reactions.
Frankincense and catechu should be avoided in pregnancy and breastfeeding due to insufficient safety data.

It also has a minor interaction with tetracycline antibiotics, which bromelain may theoretically increase in the bloodstream, though human evidence is mixed.
Rutin and myrrh both carry moderate interactions with antidiabetes drugs — both may theoretically increase the risk of low blood sugar (hypoglycemia). Myrrh also has a moderate interaction with warfarin, potentially decreasing its effectiveness.
Catechu has moderate interactions with blood pressure medications (theoretically increasing the risk of low blood pressure), theophylline and related drugs, and immunosuppressants.

Frankincense
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, Boswellia serrata might increase the levels of CYP1A2 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP1A2 enzymes.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, Boswellia serrata might increase the levels of CYP2C19 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2C19 enzymes.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, Boswellia serrata might increase the levels of CYP2C9 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2C9 enzymes.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, Boswellia serrata might increase the levels of CYP2D6 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Immunosuppressants
Theoretically, Boswellia serrata might alter the effects of immunosuppressive drugs.
Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.
Catechu
Antihypertensive Drugs
Theoretically, concomitant use with antihypertensive drugs might increase the risk of hypotension.
Catechu might lower blood pressure.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, black catechu may increase the levels and clinical effects of CYP1A2 substrates.
Animal research shows that black catechu can increase theophylline concentrations in the blood, possibly by inhibiting CYP1A2. Theophylline is a CYP1A2 substrate.
Immunosuppressants
Theoretically, catechu might interfere with immunosuppressant therapy.
Animal and in vitro studies suggest that catechu has immunomodulating effects.
Theophylline
Theoretically, black catechu may increase the levels and clinical effects of theophylline.
Animal research shows that black catechu can increase theophylline concentrations in the blood, possibly by inhibiting cytochrome P450 1A2.
Tienchi
Aspirin
Theoretically, taking Panax notoginseng concomitantly with aspirin may increase the risk of adverse effects from both products.
Animal research shows that taking Panax notoginseng extract with aspirin increases blood levels of salicylic acid by approximately 50% and blood levels of Panax notoginseng by 75% to 196%. This effect may be due to increased absorption of both products.
Caffeine
Theoretically, taking Panax notoginseng may decrease the levels and clinical effects of caffeine.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction is attributed to the ability of Panax notoginseng to increase the activity of cytochrome P450 1A2 (CYP1A2) enzymes.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.
Warfarin (Coumadin)
Theoretically, taking Panax notoginseng concomitantly with warfarin may increase the risk of bleeding.
Animal research shows that taking Panax notoginseng concomitantly with warfarin increases plasma warfarin levels, prothrombin time, and international normalized ratio when compared with control. In vitro research also suggests that Panax notoginseng may downregulate expression of cytochrome P450 3A4 enzymes, which may affect warfarin metabolism.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Myrrh
Antidiabetes Drugs
Theoretically, myrrh might increase the risk of hypoglycemia when taken with antidiabetes drugs.
In vitro and animal research suggests that myrrh has hypoglycemic effects.
Warfarin (Coumadin)
Theoretically, myrrh might decrease the effectiveness of warfarin.
In one case, a patient who was previously stable on warfarin had a significant decline in international normalized ratio (INR) following consumption of an aqueous extract of myrrh.
Rutin
Antidiabetes Drugs
Theoretically, taking rutin with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research suggests that rutin has hypoglycemic effects.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
