Acetaminophen, Caffeine, Pyrilamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Panax Notoginseng interacts with.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for this Midol-type combo: one of its ingredients is caffeine, and your body clears caffeine using a liver enzyme called CYP1A2. Panax notoginseng appears to speed that enzyme up, so in theory it could push caffeine out of your system a little faster. The practical result would be less of caffeine's pick-me-up, so the product might feel slightly weaker on the energy side. It shouldn't make the medicine dangerous.
I'll be honest, this is based only on animal lab work (in one study caffeine levels dropped about 37%), so the real-world effect in people is uncertain and likely small. The acetaminophen and pyrilamine parts aren't really the concern here. If you take this regularly, just run it by your pharmacist or doctor first.

Here's the gist: this Midol product contains caffeine, and Panax notoginseng may speed up the liver enzyme (CYP1A2) that clears caffeine out of your body. In theory, that means the caffeine could leave faster and give you a bit less of its usual lift, so you might notice it doesn't perk you up or ease that caffeine-type headache as well as expected. The acetaminophen and pyrilamine portions aren't really part of this interaction.
I'll be straight with you, though: this is based on early animal research, not people, and it used IV dosing, so it's pretty theoretical. It's not dangerous. If you take this regularly with the herb, just mention it to your pharmacist before changing anything.
