White mulberry leaf is generally well tolerated when taken by mouth. The most common side effects are digestive: bloating, flatulence (gas), loose stools, and constipation.

In one clinical study, about half of participants experienced bloating and gas, a quarter had loose stools, and a fifth had constipation, all at a dose of 4.6 grams three times daily for 4 weeks—though these effects often eased as people continued taking it. During pregnancy and breastfeeding, there isn't enough safety data for medicinal amounts of white mulberry leaf extract, so avoid taking it in these forms if you're pregnant or nursing.

Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.
