Test-Up Ingredients & Drug Interactions
by MDLogic
What is this page for?
First and foremost: checking Test-Up against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

HelloPharmacist Scorecard of Test-Up by MDLogic
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Test-Up contains 6 active ingredients. The main ones are Testofen Fenugreek extract, Forskolin (from Coleus), Oatstraw extract, Tribulus terrestris powder, plus proprietary botanical blends including Horny Goat Weed extract and Fenuside.
These are herbal ingredients traditionally used to support testosterone and sexual function. The product also contains inactive ingredients — Hypromellose, Monolaurin, and Microcrystalline Cellulose — which are common capsule fillers and binders.
Does it work?
Strong evidence
The evidence on Test-Up's ingredients is mixed. Oatstraw extract is rated Likely Effective for managing cholesterol (dyslipidemia) and heart disease.
Tribulus terrestris is rated Possibly Effective for sexual dysfunction. However, Forskolin has insufficient reliable evidence for the conditions it's studied in — angina, asthma, heart failure, eczema, and dry eye.
We don't hold effectiveness ratings for the other ingredients in the product.
How safe is it?
Well-documented data
Forskolin is generally well tolerated when taken by mouth, though some trial participants reported constipation, diarrhea, nausea, or vomiting. The data notes it may lower blood pressure and is not well studied long-term, so careful use and professional guidance are advised.
The safety data advises against Forskolin use during pregnancy and breastfeeding. Oatstraw extract is generally well tolerated and considered likely safe in pregnancy and breastfeeding when eaten as food; concentrated supplements are less studied.
You may experience bloating, flatulence, or mild gastrointestinal distention if you increase fiber intake too quickly — these usually fade with continued use. Tribulus terrestris is often well tolerated short-term but has limited long-term safety data.
Rare serious effects like liver and kidney injury or seizures have been reported. The safety data advises against Tribulus during pregnancy and breastfeeding.
Meds to double-check
Major interaction found
Stop and check with your pharmacist before taking Test-Up if you use any of these: nitrates or calcium channel blockers for your heart (Major severity), blood pressure medications, blood sugar medications or insulin, blood thinners or antiplatelet drugs, lithium, or any drug metabolized by your liver's CYP2C9 or CYP3A4 enzymes. These are Moderate-severity interactions that can reduce how well your medication works or increase side effects.
The bottom line
Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Major medication interactions have been identified, and safety information is well characterized.
Test-Up is designed to support testosterone and sexual function using herbal ingredients. It's not for you if you take nitrates, calcium channel blockers, blood pressure medications, blood sugar medications, insulin, lithium, or blood thinners — the interactions are serious enough to warrant a conversation first.
Even if you're on other medications, check them with your pharmacist before starting this product.

Test-Up by MDLogic Drug Interactions
HelloPharmacist Interaction Report
Test-Up by MDLogic contains several ingredients with documented interactions with medications.
The most serious concerns come from Forskolin, which has Major-severity interactions with nitrates (medications for chest pain) and calcium channel blockers (heart medications). Both of these drug types work by relaxing blood vessels, and adding Forskolin may boost that effect too much, risking a dangerous drop in blood pressure or heart problems.
Read the full breakdown — every affected drug type, severity by severity
Forsklin and Tribulus (another ingredient) both interact with blood pressure medications (antihypertensive drugs), blood sugar medications (antidiabetes drugs and insulin), and blood thinners (anticoagulant and antiplatelet drugs) — all rated Moderate severity. Forskolin also affects how your liver processes certain medications metabolized by CYP2C9 and CYP3A4 enzymes, which can reduce their effectiveness or raise their levels.
Tribulus has an additional Moderate interaction with lithium, a psychiatric medication that needs careful monitoring.
Oatstraw extract in this product interacts with insulin and other blood sugar medications at Moderate severity, raising the risk of low blood sugar (hypoglycemia).
We could not check Saponins, Fenuside, or Flavonoids — we hold no interaction data for these ingredients. Altogether, these interactions span 986 individual medications.
Before you start Test-Up, run your exact medications through the checker on this page.

Estrogens
Theoretically, concomitant use of horny goat weed with estrogens might increase their therapeutic and adverse effects.
In vitro evidence suggests that horny goat weed has estrogenic activity. In clinical research, horny goat weed has been shown to increase blood levels of estrogen in some females.
ForsLean Coleus forskohlii root extract
Calcium Channel Blockers
Theoretically, combining coleus with calcium channel blockers might increase the coronary vasodilatory effects.
Forskolin, a constituent of coleus, and calcium channel blockers both cause coronary vasodilatory effects.
Nitrates
Theoretically, combining coleus with nitrates might increase the coronary vasodilatory effects.
Forskolin, a constituent of coleus, and nitrates both cause coronary vasodilatory effects.
Anticoagulant/Antiplatelet Drugs
Theoretically, concomitant use of coleus and anticoagulant or antiplatelet drugs might increase the risk of bruising and bleeding.
In vitro and animal research shows that forskolin, a constituent of coleus, can inhibit platelet aggregation and adhesion.
Antihypertensive Drugs
Theoretically, combining coleus with antihypertensive drugs might cause additive blood pressure lowering effects and increase the risk of hypotension.
Animal research shows that forskolin, a constituent of coleus, may lower blood pressure.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, taking coleus may affect drugs metabolized by CYP2C9 and increase the risk of adverse effects or reduce the effectiveness.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.
Warfarin (Coumadin)
Theoretically, taking coleus may affect the metabolism of warfarin and increase the risk of adverse effects or reduce the effectiveness.
Some animal research shows that coleus extract can induce cytochrome P450 2C9 (CYP2C9), an enzyme that metabolizes warfarin. However, other in vitro research shows that coleus can inhibit CYP2C9. Theoretically, taking coleus with drugs metabolized by CYP2C9 might affect drug levels and the risk of adverse effects. Until more is known, advise patients that taking coleus might increase or decrease levels of warfarin.
Testofen Fenugreek extract
Anticoagulant/Antiplatelet Drugs
Theoretically, fenugreek might have additive effects when used with anticoagulant or antiplatelet drugs.
Some of the constituents in fenugreek have antiplatelet effects in animal and in vitro research. However, common fenugreek products might not contain sufficient concentrations of these constituents for clinical effects. A clinical study in patients with coronary artery disease or diabetes shows that taking fenugreek seed powder 2.5 grams twice daily for 3 months does not affect platelet aggregation, fibrinolytic activity, or fibrinogen levels .
Antidiabetes Drugs
Theoretically, fenugreek seed might have additive hypoglycemic effects when used with antidiabetes drugs.
Clinical research shows that fenugreek seed can reduce fasting blood glucose and 2-hour postprandial glucose levels in adults with type 2 diabetes.
Clopidogrel (Plavix)
Theoretically, fenugreek seed might alter the clinical effects of clopidogrel by inhibiting its conversion to the active form.
Animal research shows that fenugreek seed 200 mg/kg daily for 14 days increases the maximum serum concentration of clopidogrel by 21%. It is unclear how this affects the pharmacokinetics of the active metabolite of clopidogrel; however, this study found that concomitant use of fenugreek seed and clopidogrel prolonged bleeding time by an additional 11%.
Metoprolol (Toprol)
Theoretically, fenugreek seed might have additive hypotensive effects when used with metoprolol.
Animal research shows that fenugreek seed 300 mg/kg daily for 2 weeks decreases systolic and diastolic blood pressure by 9% and 11%, respectively, when administered alone, and by 15% and 22%, respectively, when given with metoprolol 10 mg/kg.
Phenytoin (Dilantin)
Theoretically, fenugreek might decrease plasma levels of phenytoin.
Animal research shows that taking fenugreek seeds for 1 week decreases maximum concentrations and the area under the curve of a single dose of phenytoin by 44% and 72%, respectively. This seems to be related to increased clearance. So far, this interaction has not been reported in humans.
Sildenafil (Viagra)
Theoretically, concurrent use of sildenafil and fenugreek might reduce levels and therapeutic effects of sildenafil.
Animal research shows that taking fenugreek seeds for 1 week reduces maximum concentrations and the area under the curve of a single dose of sildenafil by 27% and 48%, respectively. So far, this interaction has not been reported in humans.
Theophylline
Theoretically, fenugreek may reduce the levels and clinical effects of theophylline.
Animal research shows that fenugreek 50 grams daily for 7 days reduces the maximum serum concentration (Cmax) of theophylline by 28% and the area under the plasma drug concentration-time curve (AUC) by 22%.
Warfarin (Coumadin)
Theoretically, fenugreek might have additive effects with warfarin and increase the international normalized ratio (INR).
Some fenugreek constituents have antiplatelet effects, although these might not be present in concentrations that are clinically significant. In one case report, a patient taking warfarin experienced an increased INR when starting to take fenugreek in combination with boldo.
Antihypertensive Drugs
Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.
Tribulus terrestris L. Extract
Antidiabetes Drugs
Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
Antihypertensive Drugs
Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
Lithium
Theoretically, tribulus might increase the levels and clinical effects of lithium.
Tribulus is thought to have diuretic properties. Due to these potential diuretic effects, tribulus might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
Oatstraw extract
Antidiabetes Drugs
Theoretically, oats may have additive effects with antidiabetic agents and might increase the risk of hypoglycemia.
Consuming oats can decrease blood glucose in patients with diabetes. In those who require insulin, taking oats 100 grams daily for 2 days reduces the insulin dose required to achieve metabolic control.
Insulin
Concomitant use of oats and insulin might increase the risk of hypoglycemia.
In patients with insulin-dependent type 2 diabetes, taking oats 100 grams daily for 2 days reduces the insulin dose required to achieve metabolic control.
