Acetaminophen, Chlorpheniramine, Dextromethorphan, Phenylpropanolamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Let's focus on the dextromethorphan piece of this combo cold product. Dextromethorphan (the cough suppressant) is cleared by an enzyme called CYP2D6. In lab studies, 1,3-DMAA appears to block that enzyme. If that happens in real life, dextromethorphan could build up and lead to more drowsiness, dizziness, or feeling foggy and out of sorts.

Honestly though, my bigger worry here is overlap, not just enzymes. This product already contains phenylpropanolamine, a decongestant that raises blood pressure and heart rate, and 1,3-DMAA is a powerful stimulant that does the same thing. Stacking them could push your blood pressure and heart rate up to a risky level. 1,3-DMAA has been banned as unsafe, so I'd avoid it and check with your pharmacist or doctor.

1,3-DMAA is thought to have stimulant effects. There is concern that taking 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some preliminary research shows that taking 1,3-DMAA 50 mg daily in combination with caffeine 250 mg daily does not increase respiratory rate, blood pressure, or other cardiovascular outcomes compared to taking caffeine alone in healthy men. However, a number of cardiovascular side effects have been reported for patients taking 1,3-DMAA in combination with other stimulants including caffeine. Theoretically, combining 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some stimulant drugs include amphetamine, caffeine, diethylpropion (Tenuate), methylphenidate, phentermine (Ionamin), pseudoephedrine (Sudafed, others), and many others.

Here's the thing to watch with this combo cold product: it contains phenylpropanolamine, a decongestant that is itself a stimulant. It tightens blood vessels and can nudge your blood pressure and heart rate up. DMAA does the same kind of thing, only more strongly. Stacking two stimulants together can pile on those effects, so the worry is a bigger jump in blood pressure, a racing or pounding heart, jitteriness, or headache.

This concern is based mainly on how these ingredients work, not on solid studies of this exact pairing, so think of it as a sensible caution rather than a proven danger. Honestly, the simplest move is to steer clear of DMAA altogether, since the FDA has flagged it as unsafe. Please run this by your pharmacist or doctor before mixing them.
