Brompheniramine, Dextromethorphan, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the bigger picture: DMAA itself is a banned, risky stimulant that can spike your blood pressure and heart rate, so it really shouldn't be in any supplement to begin with. On top of that, lab studies suggest DMAA can block an enzyme called CYP2D6, which helps clear part of your Dimetapp DM combo. The dextromethorphan piece leans most on that enzyme, so it could build up and bring more drowsiness, dizziness, or even confusion or agitation.

There's another layer worth noting: DMAA's stimulant effect and the phenylephrine decongestant both raise blood pressure, which could stack up uncomfortably. This particular interaction is only based on test-tube data, so it's theoretical, but given DMAA's real dangers, I'd avoid it and check with your pharmacist.

1,3-DMAA is thought to have stimulant effects. There is concern that taking 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some preliminary research shows that taking 1,3-DMAA 50 mg daily in combination with caffeine 250 mg daily does not increase respiratory rate, blood pressure, or other cardiovascular outcomes compared to taking caffeine alone in healthy men. However, a number of cardiovascular side effects have been reported for patients taking 1,3-DMAA in combination with other stimulants including caffeine. Theoretically, combining 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some stimulant drugs include amphetamine, caffeine, diethylpropion (Tenuate), methylphenidate, phentermine (Ionamin), pseudoephedrine (Sudafed, others), and many others.

Here's the piece I want you to focus on: the phenylephrine inside Dimetapp DM is a decongestant that tightens blood vessels and can nudge up your blood pressure and heart rate. DMAA is itself a powerful amphetamine-like stimulant that does the very same thing. Stacking the two means you may be pushing your heart and blood vessels in the same direction at once, which could lead to a racing or pounding heartbeat, a spike in blood pressure, headache, jitteriness, or chest discomfort.

To be fair, this concern is based on how these ingredients work rather than on solid studies of this exact combo, so it's theoretical. But honestly, DMAA has been banned by the FDA as unsafe, so I'd steer clear of it regardless. Please check with your pharmacist or doctor before combining anything like this.
