Dextromethorphan, Guaifenesin, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Let me break this down for the cough and cold combo you mentioned. Of the three ingredients, dextromethorphan (the cough suppressant) is the one cleared by an enzyme called CYP2D6. In lab studies, 1,3-DMAA appears to block that enzyme. If that holds true in people, dextromethorphan could build up in your body, which may lead to more dizziness, drowsiness, confusion, or a jittery, restless feeling.

There's another wrinkle here. Both phenylephrine (the decongestant) and DMAA are stimulants that raise blood pressure and heart rate, so stacking them could push those numbers higher. Honestly, DMAA has been flagged as unsafe and pulled by the FDA, so I'd steer clear of it entirely. Please run this by your pharmacist or doctor before combining anything.

1,3-DMAA is thought to have stimulant effects. There is concern that taking 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some preliminary research shows that taking 1,3-DMAA 50 mg daily in combination with caffeine 250 mg daily does not increase respiratory rate, blood pressure, or other cardiovascular outcomes compared to taking caffeine alone in healthy men. However, a number of cardiovascular side effects have been reported for patients taking 1,3-DMAA in combination with other stimulants including caffeine. Theoretically, combining 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some stimulant drugs include amphetamine, caffeine, diethylpropion (Tenuate), methylphenidate, phentermine (Ionamin), pseudoephedrine (Sudafed, others), and many others.

The piece of this combo to watch is the phenylephrine. It's a decongestant that tightens blood vessels, which can nudge your blood pressure up and make your heart work a little harder. DMAA is a strong, amphetamine-like stimulant that does much the same thing, so stacking the two could push those effects further than either one alone. Realistically that might show up as a pounding or racing heartbeat, headache, jitteriness, or a noticeable rise in blood pressure.

Honestly, the bigger issue here is DMAA itself. The FDA has pulled it from supplements because of reports of dangerous heart events. The interaction is based on theory and pharmacology rather than strong studies, but I'd steer clear of DMAA entirely. Please check with your pharmacist or doctor before mixing them.
