Dextromethorphan, Pseudoephedrine, Triprolidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Let's talk about Actifed DM, which is a combo of dextromethorphan (a cough suppressant), pseudoephedrine (a decongestant), and triprolidine (an antihistamine). The piece that overlaps with DMAA here is dextromethorphan, which your liver clears using an enzyme called CYP2D6. In lab studies, DMAA appears to block that enzyme. If that holds true in people, dextromethorphan could build up and cause more dizziness, drowsiness, confusion, or jitteriness.

There's a bigger concern, though. DMAA is a powerful stimulant, and pseudoephedrine is also a stimulant decongestant. Stacking them can really push up your blood pressure and heart rate. Honestly, DMAA has been banned as unsafe, so the safest move is to avoid it entirely and check with your pharmacist or doctor.

1,3-DMAA is thought to have stimulant effects. There is concern that taking 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some preliminary research shows that taking 1,3-DMAA 50 mg daily in combination with caffeine 250 mg daily does not increase respiratory rate, blood pressure, or other cardiovascular outcomes compared to taking caffeine alone in healthy men. However, a number of cardiovascular side effects have been reported for patients taking 1,3-DMAA in combination with other stimulants including caffeine. Theoretically, combining 1,3-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some stimulant drugs include amphetamine, caffeine, diethylpropion (Tenuate), methylphenidate, phentermine (Ionamin), pseudoephedrine (Sudafed, others), and many others.

Here's the thing about Actifed DM: one of its ingredients, pseudoephedrine, is itself a stimulant decongestant. It already nudges your heart rate and blood pressure upward. DMAA is a powerful, amphetamine-like stimulant that does the very same thing. Stacking the two means you're hitting your heart and blood vessels with a double dose of that 'fight or flight' push.

What could that feel like? A racing or pounding heart, jitteriness, headache, and higher blood pressure. While this concern is mostly theoretical and based on how these stimulants work, real cardiovascular problems have been reported with DMAA. On top of that, DMAA has been banned as unsafe, so I'd steer clear of it entirely. Please check with your pharmacist or doctor before combining anything like this.
