CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Acetaminophen, Caffeine, Pyrilamine
Acetaminophen, Caffeine, Pyrilamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical takeaway for your Midol Max Strength, which combines acetaminophen, caffeine, and the antihistamine pyrilamine. Aegeline has been shown to block an enzyme called CYP3A4 in test-tube studies. In theory, slowing that enzyme could let certain medicines build up a bit higher than usual. None of these three ingredients are prodrugs, so they wouldn't lose their effect, the concern would be slightly stronger effects.
I want to be straight with you though: this interaction has only been seen in lab dishes, never reported in actual people, and it's rated minor. So I wouldn't lose sleep over it. My bigger worry is aegeline itself, since it's been tied to serious liver injury and regulators warned against it. Honestly, the safest move is avoiding aegeline altogether. Please check with your pharmacist or doctor.

About Aegeline
What is Aegeline?
- Weight loss
- Fat burning / metabolism support
- Sports and bodybuilding supplements
- General 'energy' products
Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.
