Tisotumab Vedotin-Tftv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Let me put this one in perspective. Tivdak is an antibody-drug conjugate given by IV in a cancer clinic, and part of it (a chemotherapy payload called MMAE) is cleared with help from the liver enzyme CYP3A4. Aegeline has been shown in test-tube studies to slow that same enzyme. In theory, blocking CYP3A4 could let more of that toxic payload hang around, which might mean more bruising or bleeding, nerve tingling or numbness, eye irritation, nausea, or low blood counts.

That said, this is lab-only data and has never been seen in real patients, so I wouldn't lose sleep over it. The bigger issue is aegeline itself, which has been tied to serious liver injury and is one many people should simply avoid. Please loop in your oncology team before using any supplement during this treatment.
