Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the short version: there's only a small, very theoretical concern about pairing atractylodes with aliskiren (Tekturna). In high-dose animal studies, atractylodes slowed down an enzyme (CYP3A4) that helps your body process certain medications. Aliskiren is partly handled by this same enzyme, and it works as-is (it's not a prodrug that needs activating). So if that enzyme were blocked, aliskiren levels could drift up a bit, which in theory might mean more of its usual effects, like lower blood pressure, dizziness, or lightheadedness.

That said, this has only been seen in lab and animal research, never in people, so I wouldn't lose sleep over it. Still, it's worth a quick mention to your pharmacist or doctor before starting atractylodes, especially if your blood pressure already runs low.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
