Trandolapril, Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

Here's the gist with Tarka (the trandolapril plus verapamil combo) and atractylodes. In animal studies given large doses, atractylodes seemed to speed up a liver enzyme called CYP1A2. If that happened in people, it could in theory clear certain medications a bit faster, making them slightly less effective. For Tarka, that might mean a touch less blood pressure control, so things like a faster heart rate or higher readings rather than new side effects.

Honestly though, this is theoretical and only seen in lab and animal work, never confirmed in humans, and it's rated minor. Verapamil mostly relies on a different enzyme (CYP3A4) anyway. I wouldn't lose sleep over it, but if you take Tarka, please check with your pharmacist or doctor before starting atractylodes.

Trandolapril, Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Tarka is a combination pill that pairs trandolapril (a blood pressure medicine) with verapamil. The verapamil part is processed by a liver enzyme called CYP3A4, and that is where atractylodes comes into the picture. In lab and animal studies, high doses of atractylodes seemed to slow down a similar enzyme. If that happened in people, it could let verapamil build up a bit, which might mean a lower heart rate, dizziness, swollen ankles, or constipation.

Here is the honest part: this has only been seen in animals at high doses, never in humans, so it is mostly theoretical and rated minor. I wouldn't lose sleep over it, but since blood pressure and heart rate matter, please run it by your pharmacist or doctor first.
