Eliglustat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist with Cerdelga (eliglustat). Your body relies on a liver enzyme called CYP3A4 to help clear this medication out. Ayahuasca contains beta-carboline compounds that, in lab studies, appear to slow that same enzyme down. If that happened in a person, eliglustat could build up to higher levels than intended.

Why that matters: eliglustat at higher levels can affect your heart's rhythm (it can lengthen the QT interval), and you might notice more headache, dizziness, fatigue, or stomach upset. The honest part, though, is that this is theoretical, based only on test-tube data, and hasn't been shown in actual patients. Still, eliglustat is a drug where blood levels really matter, so please don't combine these without checking with your doctor or pharmacist first.

About Eliglustat

What Is Eliglustat Used For?

Brand name: Cerdelga

Cerdelga (eliglustat) is used for the long-term treatment of Gaucher disease type 1 in adults. Gaucher disease type 1 is a genetic condition in which a fatty substance called glucosylceramide builds up in organs like the liver, spleen, and bone marrow, causing them to enlarge and leading to problems like anemia and low platelet counts.

Cerdelga is only appropriate for adults whose bodies process the drug in a predictable way — specifically those identified as CYP2D6 extensive metabolizers (EMs), intermediate metabolizers (IMs), or poor metabolizers (PMs) by an FDA-cleared genetic test. People who are CYP2D6 ultra-rapid metabolizers may not reach drug levels high enough to benefit, and Cerdelga is not recommended for them.
