Finerenone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for Kerendia (finerenone): your body relies on a liver enzyme called CYP3A4 to clear it out. Ayahuasca contains beta-carboline compounds that, at least in lab studies, can slow that same enzyme down. In theory, that could let finerenone build up higher than intended in your blood.

Why does that matter? Finerenone's main risk is raising potassium levels (hyperkalemia), which can cause muscle weakness, an irregular heartbeat, or in severe cases dangerous heart rhythms. More drug hanging around could nudge that risk up. Keep in mind this is only a theoretical, lab-based concern that hasn't been confirmed in people. Still, ayahuasca carries its own serious dangers, so please talk with your doctor or pharmacist before mixing anything with your kidney medication.

What Is Finerenone Used For?

Brand name: Kerendia

Kerendia (finerenone) is used in adults with chronic kidney disease (CKD) linked to type 2 diabetes. It helps reduce the risk of kidney function decline, end-stage kidney disease, cardiovascular death, non-fatal heart attack, and hospitalization for heart failure in these patients.

Kerendia is also used in adults with heart failure who have a left ventricular ejection fraction (LVEF) — a measure of how well the heart pumps — of 40% or higher. In this group, it reduces the risk of cardiovascular death, hospitalization for heart failure, and urgent heart failure visits.
