CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Guaifenesin, Hydrocodone Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with this combo. Hydrocodone is actually a prodrug, meaning your liver enzyme called CYP2D6 turns part of it into hydromorphone, a stronger painkiller. Lab studies suggest the beta-carboline compounds in ayahuasca (harmine and harmaline) can block CYP2D6. If that blocking happens, less hydrocodone gets converted, so you might get less pain and cough relief than expected. The guaifenesin part just thins mucus and isn't really involved here.
That said, this is theoretical and only seen in test-tube research, not in real people, so don't panic. The bigger worry with ayahuasca is its strong MAOI activity mixing with opioids. Please talk with your pharmacist or doctor before combining these.

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your cough syrup that pairs guaifenesin with hydrocodone. Hydrocodone is partly handled by a liver enzyme called CYP3A4. In lab studies, the beta-carboline compounds in ayahuasca can slow that enzyme down. If they do the same thing in a real person, your body might clear hydrocodone more slowly, letting it build up. That could mean stronger opioid effects like heavy drowsiness, slowed breathing, dizziness, constipation, or confusion.
I want to be honest though: this is only seen in test-tube research, not in actual people, so it stays theoretical for now. There's a bigger worry too. Ayahuasca acts like an MAOI, and mixing MAOIs with opioids can be dangerous on its own. Please check with your doctor or pharmacist before combining these.
