Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for Camzyos (mavacamten). Your body relies on a liver enzyme called CYP3A4 to help clear this medicine. Lab studies suggest the beta-carboline compounds in ayahuasca can slow that enzyme down. If that happened in a person, mavacamten could build up to higher levels than intended.

Why that matters: mavacamten works by easing how forcefully your heart squeezes, so too much can drop your heart's pumping function too far, leading to shortness of breath, swelling, dizziness, or even heart failure symptoms. This drug is closely dose-monitored for that reason. That said, this interaction is only theoretical, based on test-tube data and never confirmed in humans. Please don't change anything on your own. Run it by your cardiologist or pharmacist first.

“Grapefruit is the first thing I would set aside for anyone taking mavacamten (Camzyos), the medicine for obstructive hypertrophic cardiomyopathy. It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”
