Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the thing about Paxlovid: it already contains ritonavir on purpose, specifically to block the CYP3A4 enzyme so the antiviral part (nirmatrelvir) stays in your body longer and works better. Lab studies suggest the beta-carboline alkaloids in ayahuasca can also slow down that same CYP3A4 enzyme. So in theory, you'd be stacking one blocker on top of another, which could nudge nirmatrelvir levels higher and lead to more side effects like nausea, diarrhea, an off or metallic taste, or muscle aches.

I'll be honest, this concern comes from test-tube research only and hasn't been shown in real people, so it's more cautionary than proven. Honestly, my bigger worry is that ayahuasca itself can be dangerous and is often illegal. Please run this by your pharmacist or doctor before mixing the two.
