Thioridazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the concern with this pairing. Your body clears thioridazine largely through a liver enzyme called CYP2D6. The beta-carboline compounds in ayahuasca (harmine and harmaline) appear to block that enzyme, at least in lab studies. If that happens in a person, thioridazine could build up to higher levels than expected, and that matters a lot with this particular drug.

Thioridazine is well known for affecting heart rhythm (it can prolong the QT interval), and higher levels raise that risk along with drowsiness, dizziness, and low blood pressure. So far this is only seen in test-tube research, not in real patients, so we can't be sure how strong it is. Still, given thioridazine's serious heart risks, please don't combine these without first talking to your doctor or pharmacist.

What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
