Why this applies to Irinotecan

Irinotecan falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might alter the levels and clinical effects of OATP substrates.
Some pharmacokinetic research shows that baicalin, a constituent of Baikal skullcap, can decrease plasma levels of rosuvastatin. The mechanism is thought to involve stimulation of the activity of the organic anion-transporting polypeptide 1B1 (OATP1B1), which transports rosuvastatin into the liver. This decreases plasma levels of the drug, but increases levels at the site of action in the liver. The degree to which rosuvastatin levels are affected depends on the OATP1B1 haplotype of the individual. Baikal skullcap might also affect other OATP1B1 substrates.

Here's the gist with irinotecan (Camptosar) and Baikal skullcap. Irinotecan relies on a liver transporter called OATP1B1 to move it (and its active form, SN-38) in and out of liver cells. Baikal skullcap, mainly its compound baicalin, may speed up that transporter. In theory, that could shift where the drug ends up, possibly lowering blood levels while pushing more into the liver. This raises a couple of concerns: a small dip in how well the chemo works, or extra strain on the liver alongside irinotecan's usual issues like diarrhea, low blood counts, and nausea.

Honestly, this is mostly lab and limited human data, borrowed from a statin study, so we can't say exactly what happens with irinotecan. With a cancer drug, I wouldn't gamble. Please run this by your oncology team before using the herb.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications. Irinotecan lipid complex is in a class of antineoplastic medications called topoisomerase I inhibitors. It works by stopping the growth of cancer cells.
