Bitter Almond and Codeine Phosphate

Here is the interaction we found between these two, with the evidence behind it.

CNS DEPRESSANTS

Why this applies to Codeine Phosphate

Codeine Phosphate falls under CNS DEPRESSANTS — the group Bitter Almond interacts with.

Theoretically, bitter almond oil may have additive effects with other CNS depressants.
Bitter almond and its oil contain amygdalin, which is hydrolyzed to toxic hydrocyanic acid. This can cause CNS depression and respiratory failure.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic worry with mixing bitter almond and codeine. Codeine is an opioid, and one of its main effects is calming down the brain and slowing your breathing. Bitter almond contains amygdalin, which your body can turn into cyanide, and in larger amounts that can also depress the nervous system. Stacked together, the theory is they could pile on each other, leaving you overly sleepy, foggy, or breathing too slowly.

I want to be honest that this concern is theoretical, based on how these chemicals behave rather than real-world reports. The bigger issue, frankly, is that bitter almond itself is genuinely toxic and isn't safe to take internally at all. Please skip it, and check with your pharmacist or doctor if you have questions.

From our pharmacist’s overview on the Codeine Phosphate page

“Codeine phosphate is an opioid painkiller and cough suppressant that slows breathing along with brain activity, and the wrong supplement can push that effect into dangerous territory. Seven of the 144 documented ingredients rate major, and nearly all of that serious risk comes from things that deepen sedation.”

About Bitter Almond

What is Bitter Almond?

Bitter Almond
  • Cough suppressant
  • Muscle spasms (antispasmodic)
  • Itching and skin conditions
  • Flavoring agent
  • Sedative or 'narcotic' effects

Bitter almond is a variety of almond that contains amygdalin, a compound the body converts into cyanide, making it potentially poisonous. Because of this serious toxicity risk, bitter almond should not be eaten or used internally, and there is no reliable proof it treats any health condition. If you want almond's nutrition, choose ordinary sweet almonds instead.

About Codeine

What Is Codeine Used For?

Brand name: Codeine

Codeine Sulfate tablets are used for the management of mild to moderate pain in adults and adolescents when other treatments have not worked well enough or are not tolerated. It is an opioid medication, so it is only appropriate when non-opioid options are inadequate.

Because of its serious risks — including addiction, misuse, and overdose — codeine is reserved for patients who truly need an opioid. Prescribers are expected to use the lowest effective dose for the shortest time possible.
