Bitter almond and its oil contain amygdalin, which is hydrolyzed to toxic hydrocyanic acid. This can cause CNS depression and respiratory failure.

This combination pill contains codeine, which already slows your breathing and makes you drowsy. The concern here is mostly about safety, not how your liver processes the medicine. Bitter almond contains amygdalin, which the body can turn into cyanide, a real poison that can depress the nervous system and breathing on its own. So in theory, pairing bitter almond with codeine could stack two things that quiet your brain and slow your breathing.

Keep in mind this is a theoretical, pharmacology-based caution, not something proven in studies. Still, the simplest and safest move is to avoid bitter almond entirely (regular sweet almonds in food are fine). If you have questions, check with your pharmacist or doctor before combining them.
