Bitter Orange + Clomipramine
Clomipramine brand names: Anafranil, Anafranil SR.
Clomipramine falls into 2 drug categories that Bitter Orange can interact with; the most serious is rated moderate. Bitter orange might increase levels of drugs metabolized by CYP3A4.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Bitter Orange interacts with whole categories of medication. Clomipramine falls into 2 of those categories, so it can interact with Bitter Orange in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Clomipramine
Clomipramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for clomipramine (Anafranil): bitter orange may block an enzyme in your gut called CYP3A4 that helps process this medication. Since clomipramine is active as-is (not a prodrug that needs activating), slowing its breakdown could let a bit more of it build up in your system. In theory that might nudge up its usual side effects, things like dry mouth, drowsiness, constipation, dizziness, or a faster heartbeat.
Honestly, the evidence here is modest and based on freshly squeezed bitter orange juice, so we don't know how much concentrated weight-loss supplements would matter. Bitter orange is also a stimulant on its own, which is worth keeping in mind. Please check with your pharmacist or doctor before combining the two.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Clomipramine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for you: your body uses an enzyme called CYP2D6 to help clear clomipramine out of your system. Bitter orange contains a compound (octopamine) that, in test-tube studies, mildly slows that same enzyme down. If that effect happened in real people, clomipramine could build up a bit and you might feel more of its usual side effects, like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat.
The honest part: this is only theoretical so far, seen in lab dishes and never actually reported in humans, so it's a minor concern. But bitter orange itself is a stimulant that can raise heart rate and blood pressure, which already overlaps with clomipramine's effects on the heart. I'd run it by your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Clomipramine page
“A tricyclic antidepressant used mostly for OCD, clomipramine (Anafranil) is unforgiving of shifts in its blood level, which is why its 236 supplement interactions deserve genuine wariness. Most of the list involves metabolism: herbs that speed up or slow down the liver enzymes handling clomipramine can drag its level too low or push it…”

About Bitter Orange
What is Bitter Orange?
- Weight loss
- Athletic performance and energy
- Digestive upset
- Appetite control
- Aromatherapy for stress (neroli oil)
Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. Evidence that it works for weight loss or performance is limited, and because it can raise heart rate and blood pressure, people with heart problems, high blood pressure, or who are pregnant should avoid it. Talk to your pharmacist or doctor before using it, especially if you take other stimulants.

Natural Medicines records Clomipramine as a medicine that can lower these. This is not an interaction with Bitter Orange — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
