Here's the gist for desipramine: like other tricyclic antidepressants, it can stretch out the heart's electrical recovery time, what we call the QT interval. Bitter orange, thanks to its stimulant synephrine, may nudge that same effect along while also bumping up your heart rate and blood pressure. Stacked together, the worry is an irregular heartbeat, though this is mostly theoretical and rests on a single case report involving stimulants, so I don't want to scare you.

Still, since desipramine already carries some heart-rhythm risk on its own, I'd lean cautious. If you're considering a weight-loss or energy supplement with bitter orange, please run it by your pharmacist or doctor first, especially if you have any heart issues or high blood pressure.

Desipramine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, bitter orange might increase levels of drug metabolized by CYP2D6.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down desipramine and clear it out. Bitter orange contains a compound (octopamine) that may slightly slow that enzyme down. If it does, desipramine could build up a bit more than usual, which in theory could nudge up its typical side effects, things like dry mouth, constipation, drowsiness, a faster heartbeat, or feeling lightheaded when you stand.

I want to be honest about the evidence though. This has only been seen in lab (test tube) studies, never confirmed in real people, and it's rated a minor interaction. On top of that, bitter orange is a stimulant that can raise heart rate and blood pressure on its own. Since desipramine can also affect the heart, I'd run this combo past your pharmacist or doctor before adding any bitter orange supplement.

What Is Desipramine Used For?

Brand names: Norpramin, Desipramine, Pertofrane

Desipramine hydrochloride tablets — sold under the brand names Norpramin, Desipramine HCL, and Desipramine Hydrochloride — are FDA-approved for the treatment of depression in adults. Norpramin tablets and the generic formulations share this same approved indication.

Desipramine tablets are not approved for use in children. Adolescents and elderly patients may be prescribed it in certain circumstances, but at lower doses and under close supervision.
