Here's the thing about sotalol: one of its main jobs is to affect your heart's electrical rhythm, and in doing so it stretches out a part of your heartbeat called the QT interval. That effect is exactly why sotalol can sometimes trigger a dangerous irregular rhythm on its own. Bitter orange contains synephrine, a stimulant that nudges up your heart rate and blood pressure, and the worry is that stacking a stimulant on top of a QT-stretching heart medication could add to that strain.

I'll be honest, the evidence here is thin. It comes mostly from a single case report involving bitter orange plus caffeine, so this is more theoretical than proven. Still, because sotalol is a drug where rhythm changes really matter, I'd skip stimulant-based weight-loss or energy products and check with your pharmacist or doctor first.
