Lepirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Borage interacts with.

Theoretically, borage seed oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In healthy individuals, borage seed oil supplementation does not seem to affect platelet aggregation. However, gamma-linolenic acid, a constituent of borage seed oil, seems to decrease platelet aggregation by 45% and increase the risk of bleeding by 40% in animal and clinical research.

Lepirudin (Refludan) is a powerful blood thinner given by IV in the hospital, usually for people who can't take heparin. It works directly to block clotting. Borage seed oil isn't a blood thinner itself, but its main ingredient, GLA, may make your platelets (the cells that help form clots) a little less sticky. Stacking that on top of lepirudin could, in theory, nudge your bleeding risk a bit higher.

Honestly, this is mostly theoretical and based on weaker animal and clinical data, not strong human studies. Since lepirudin is given in a closely monitored hospital setting, your care team is already watching for bleeding like bruising, nosebleeds, or blood in urine. Just be sure to tell them about any borage product you take so they can keep an eye out.
