Acetaminophen, Phenylpropanolamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the situation with this combo product (which pairs acetaminophen with the decongestant phenylpropanolamine). CBD can slow down a liver enzyme called CYP1A2. We've seen this clearly with caffeine, where CBD raised caffeine levels in the blood. So in theory, a drug cleared by that same pathway could build up a bit more than usual.

That said, acetaminophen isn't mainly handled by CYP1A2, so the real-world effect on this product is likely small. Neither of these ingredients is a prodrug, so we're not flipping anything here. Still, if you take CBD regularly, it's worth a quick chat with your pharmacist, especially to avoid stacking extra acetaminophen from other cold or pain products.

In vitro research shows that cannabidiol inhibits uridine diphosphoglucuronosyl transferase (UGT) 1A9 and UGT2B7, enzymes responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs.

Here's the gist for your Tetra Caps, which pair acetaminophen with the decongestant phenylpropanolamine. Your body clears acetaminophen partly through a process called glucuronidation. In lab (test tube) studies, CBD slowed down two of those clearing enzymes (UGT1A9 and UGT2B7). On paper, that could mean acetaminophen hangs around a little longer and builds up slightly, which over time and at high doses is mostly a concern for your liver.

I want to be straight with you: this is theoretical and based only on lab work, not real people, so I wouldn't lose sleep over it. Still, since acetaminophen and your liver don't love big doses, keep within label limits and check with your pharmacist or doctor before combining them regularly.

Here's the gist of this one. The acetaminophen part of your Tetra Caps is partly broken down by a liver enzyme called CYP2E1. In lab dish studies, CBD has been shown to slow that enzyme down. In theory, if CYP2E1 is blocked, a bit more acetaminophen could linger or shift the way your liver processes it. That's worth knowing because, with acetaminophen, the liver is exactly the organ you want to protect.

Now, the honest part: this has only been seen in test tubes, never confirmed in real people, so I wouldn't lose sleep over an occasional dose. Still, if you use concentrated CBD products regularly, or you ever take more than the labeled acetaminophen amount, please check with your pharmacist or doctor first so they can keep an eye on your liver.
