Why this applies to Anagrelide

Anagrelide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the simple version: your body clears anagrelide partly through a liver enzyme called CYP1A2, and CBD can slow that same enzyme down. When the enzyme works slower, anagrelide can build up a little more in your system than expected. Anagrelide is an active drug (not a prodrug), so higher levels mean a stronger effect and a higher chance of side effects.

With anagrelide, that could show up as a pounding or racing heartbeat, headache, dizziness, fluid retention, or stomach upset. The evidence here is solid that CBD blocks this enzyme, but the real concern is mostly with concentrated CBD products, not tiny amounts. Please don't change anything on your own. Check with your pharmacist or doctor first, and let them know if you notice new symptoms.

Both Agrylin and generic anagrelide capsules are used to treat thrombocythemia secondary to myeloproliferative neoplasms — a condition where bone marrow disorders cause dangerously high platelet counts. The goal is to bring platelet counts down, lower the risk of dangerous blood clots (thrombosis), and ease related symptoms including bleeding and clotting events.
