Why this applies to Chlorpropamide

Chlorpropamide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the gist with chlorpropamide and CBD. In a test tube, CBD slows down a liver enzyme called CYP2D6, which is one pathway your body uses to clear certain drugs. The thinking is that if that pathway slows, chlorpropamide could build up a bit and push your blood sugar lower than expected. Chlorpropamide is not a prodrug, so more drug hanging around means a stronger effect, not a weaker one.

What that could look like in real life is low blood sugar: shakiness, sweating, dizziness, confusion, or feeling unusually hungry. Honestly, this is theoretical, and a human study found CBD didn't affect another CYP2D6 drug. So I wouldn't panic. Just check in with your pharmacist or doctor before adding CBD, and keep an eye on your sugar readings.
