Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

Here's the simple version: this cough syrup contains codeine, an opioid that can make you drowsy. CBD, especially at higher supplement doses, may also cause sleepiness and a calming effect. When you stack two things that both slow down the brain and nervous system, those effects can pile on top of each other. So you might feel more sedated, foggy, dizzy, or short of breath than you would from the cough medicine alone.

This concern is mostly theoretical, based on how each one behaves rather than strong human studies, so it's not a reason to panic. Still, I'd play it safe: don't drive until you know how the combo hits you, and check with your pharmacist or doctor before pairing them, especially if you take other sleepy medicines.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the wrinkle with this combo: codeine is a prodrug. It doesn't do much on its own. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves your cough and pain. The guaifenesin part just loosens mucus and isn't really involved here.

In lab studies, CBD can slow down CYP2D6. If that happens, you'd likely make less morphine from your codeine, so you might get weaker cough or pain relief, not stronger side effects. Keep in mind this is mostly theoretical, and one human study found CBD didn't block CYP2D6 the way the lab predicted. So the real-world impact may be small. Still, if your cough medicine seems less effective, check in with your pharmacist or doctor.
