Darunavir falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the thing to keep in mind with this pairing. Darunavir (Prezista) is almost always taken with a booster like ritonavir or cobicistat, and those boosters are strong blockers of a liver enzyme called CYP3A4. CBD happens to be broken down by that same enzyme. So when you slow that enzyme down, CBD can hang around in your body longer and build up to higher levels than expected.

In plain terms, the CBD side of things may get stronger, not your HIV medicine. That could mean more drowsiness, diarrhea, fatigue, or changes in your liver enzymes. This is a theoretical concern based on how the drugs work, not something proven in studies, so don't panic. Still, since your HIV regimen is too important to mess with, check with your pharmacist or doctor before adding any CBD product.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the short version: your body clears darunavir (Prezista) partly through a liver enzyme called CYP3A4. CBD can slow that enzyme down, so the darunavir may not be broken down as quickly and could build up a bit more in your blood. That's not a prodrug situation here, darunavir is already the active medicine, so higher levels mean a stronger drug effect and a greater chance of side effects like nausea, diarrhea, headache, or rash.

One wrinkle: your darunavir is already taken with a booster (ritonavir or cobicistat) that strongly blocks this same enzyme, so adding CBD is a layered effect worth a real conversation. Please don't change anything on your own. Loop in your pharmacist or HIV provider before starting CBD so they can keep an eye on you.

About Darunavir

What Is Darunavir Used For?

Brand name: Prezista

Darunavir treats HIV-1 infection in adults and in children who are at least 3 years old and weigh at least 10 kg (about 22 lbs). It is not a cure for HIV, but it is used as part of a combination antiretroviral regimen to control the virus.

Darunavir must always be taken together with ritonavir — that pairing is required for darunavir to work properly. Both Prezista (brand name) and generic darunavir tablets are approved for this use. Prezista is also available as an oral suspension, which is particularly useful for younger or smaller patients.
