Dextromethorphan, Pseudoephedrine, Triprolidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

This is one of those interactions that looks worrisome on paper but mostly fizzles out in real life. In the test tube, CBD can slow down an enzyme called CYP2D6, which helps your body break down the dextromethorphan (the cough suppressant) in Actifed DM. In theory, blocking that enzyme could let dextromethorphan build up, leading to more drowsiness, dizziness, or feeling foggy.

Here's the reassuring part: an actual human study found that CBD did not meaningfully affect dextromethorphan levels. So this concern stays mostly theoretical. Still, since CBD products vary in strength and Actifed DM already causes sleepiness on its own, it's worth a quick chat with your pharmacist before pairing them, especially before driving.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the simple version for Actifed DM (dextromethorphan, pseudoephedrine, triprolidine): CBD can slow down a liver enzyme called CYP3A4. The dextromethorphan piece (the cough suppressant) leans on that enzyme to be cleared, so adding CBD may let it build up a bit more than usual. If that happens, you might notice extra drowsiness, dizziness, or feeling foggy. None of these ingredients are prodrugs, so the worry is stronger effects, not weaker ones.

This isn't usually dangerous for a short cold remedy, but it's worth a quick chat with your pharmacist or doctor, especially if you'd be using CBD regularly or feel more sleepy than expected. Don't stop anything on your own.
