Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for your HIV medicine. Doravirine (Pifeltro) is cleared from your body mainly by a liver enzyme called CYP3A4, and CBD can slow that enzyme down. When that pathway is blocked, doravirine isn't broken down as quickly, so its levels in your blood can climb a bit higher than expected. Doravirine is an active drug (not a prodrug), so more of it sticking around means a stronger effect and potentially more side effects like headache, nausea, dizziness, odd dreams, or trouble sleeping.

The evidence here is solid in general for CBD blocking this enzyme, though it hasn't been studied directly with doravirine. Because keeping HIV treatment steady really matters, please loop in your pharmacist or doctor before adding any CBD product.

About Doravirine

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used to treat HIV-1 infection in adults and in children and adolescents who weigh at least 35 kg (about 77 lbs). It is always used in combination with other antiretroviral medicines — never on its own.

Pifeltro is approved for two groups: people who have never taken HIV treatment before, and people who are already virologically suppressed (meaning HIV is undetectable in their blood) on a stable regimen and need to switch, provided they have no history of treatment failure or known resistance to doravirine.
