Why this applies to Fenfluramine

Fenfluramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the short version on pairing CBD with fenfluramine. Your liver uses an enzyme called CYP2D6 to help break fenfluramine down. In lab dish studies, CBD can slow that enzyme. If it did the same in real life, fenfluramine could build up a bit and bring on more of its usual side effects, things like reduced appetite, tiredness, dizziness, or, importantly, effects on heart rate and blood pressure.

But take a breath, because this is mostly theoretical. A human study using another CYP2D6 drug showed CBD didn't actually slow it down. So the real-world risk here looks low. Still, fenfluramine needs careful heart monitoring, so don't add CBD on your own. Run it by your child's doctor or pharmacist first.

What Is Fenfluramine Used For?

Brand name: Pondimin

Fintepla (fenfluramine oral solution) is approved to treat seizures associated with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It is used alongside other antiseizure medications.

Both conditions are severe, often hard-to-control forms of epilepsy that typically begin in childhood. Fintepla is available only through a restricted program called the Fintepla REMS because of serious heart-related risks.
