Imatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the gist of this one: imatinib (Gleevec) can act as a mild blocker of an enzyme called CYP3A4, which is one of the main pathways your body uses to clear CBD. So if you take CBD alongside imatinib, the CBD may break down a little slower and build up to higher levels than expected. That could mean more of CBD's typical effects, like drowsiness, fatigue, diarrhea, or changes in appetite.

I'll be honest with you, though, this is a theoretical concern based on how the two are processed, not something proven in real patients. It's worth a quick conversation with your oncology team before adding CBD, since imatinib is a serious cancer medicine and they'll want to keep an eye on how you feel and on your liver labs. Please don't stop or change your imatinib on your own.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for imatinib (Gleevec): your liver relies on an enzyme called CYP3A4 to help break this drug down. CBD can slow that enzyme, which means imatinib may clear out of your body more slowly and build up to higher levels than expected.

Imatinib is active on its own (not a prodrug), so higher levels can mean stronger effects and more side effects. With imatinib, that could look like more nausea, swelling around the eyes or in the legs, muscle cramps, fatigue, or changes in your blood counts. This isn't a reason to panic, and food amounts aren't the issue here, but concentrated CBD products are. Please loop in your oncology pharmacist or doctor before combining them so they can watch your dosing and labs.

What Is Imatinib Used For?

Brand name: Gleevec

Imatinib treats a range of blood cancers and solid tumors. It is used for Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML) in adults and children — whether newly diagnosed or in more advanced phases — and for Philadelphia chromosome positive acute lymphoblastic leukemia (Ph+ ALL) in both adults and pediatric patients.

It is also used for Kit-positive gastrointestinal stromal tumors (GIST), including after surgical removal, and for several rarer conditions: myelodysplastic/myeloproliferative diseases (MDS/MPD) linked to specific gene changes, aggressive systemic mastocytosis (ASM), hypereosinophilic syndrome (HES) and/or chronic eosinophilic leukemia (CEL), and unresectable, recurrent, or metastatic dermatofibrosarcoma protuberans (DFSP).
