Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Opsynvi combines two active medicines, macitentan and tadalafil, and both are handled in part by a liver enzyme called CYP2C9. Here's the thing about CBD: it can slow that enzyme down. When the enzyme works slower, these drugs can hang around in your body longer than intended, and their levels may creep up.

In practice, that usually means a bigger chance of the side effects you already know from this combo, things like low blood pressure, dizziness or lightheadedness (especially standing up), headache, flushing, or swelling. This is graded as a real, likely interaction, so it's worth a quick heads-up. Please don't stop or adjust anything on your own. Just loop in your pharmacist or doctor before adding CBD so they can keep an eye on your blood pressure and how you feel.

Opsynvi is a combo of two medicines, and both macitentan and tadalafil are cleared partly by a liver enzyme called CYP3A4. CBD can slow that enzyme down, so both drugs may build up a bit higher in your body than usual. Neither one is a prodrug, so more drug simply means stronger effects, and that's the concern here.

Practically, that could mean more low blood pressure symptoms like dizziness, lightheadedness, flushing, headache, or a stuffy nose, plus tadalafil effects such as muscle aches or back pain. This is backed by solid human data with other CYP3A4 medicines, so it's worth taking seriously. Please don't stop or change anything on your own. Check with your pharmacist or doctor first, especially since product strength varies.
