Why this applies to Mexiletine

Mexiletine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the gist for mexiletine. Your liver uses an enzyme called CYP2D6 to help clear this medicine from your body. In lab studies, CBD appears to slow that same enzyme down. If that happened in real life, mexiletine could build up a bit higher than usual, which might mean more of its typical side effects like nausea, dizziness, tremor, blurred vision, or in rare cases changes to your heart rhythm.

I want to be honest about how shaky this evidence is. It's only theoretical, based on test-tube work, and a human study with a similar CYP2D6 drug showed CBD didn't actually block it. Still, mexiletine is a heart-rhythm drug with a narrow safety window, so I'd play it safe. Please check with your pharmacist or doctor before pairing them.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the gist: your liver uses an enzyme called CYP1A2 to help clear mexiletine out of your system. CBD can slow that enzyme down, so mexiletine may break down more slowly and build up to higher levels than usual. Since mexiletine is an active drug (not a prodrug), more of it in your blood means a stronger effect and a higher chance of side effects.

With mexiletine, that could look like nausea, dizziness, tremor, lightheadedness, or in some cases new or worsening heart rhythm problems. Because mexiletine has a narrow safety window, even modest changes matter. Please don't stop or adjust either one on your own. Loop in your pharmacist or cardiologist first, since they may want to watch how you feel or check levels.

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
