Why this applies to Ondansetron

Ondansetron falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the gist with CBD and ondansetron (Zofran). Your liver uses an enzyme called CYP2D6 to help clear ondansetron. In lab (test tube) studies, CBD can slow that enzyme down. In theory, that could let a little more ondansetron hang around, which might mean more of its usual side effects, things like headache, constipation, or feeling tired.

I want to be honest about how shaky this is, though. It's theoretical, based on lab work, not real patients. A human study even showed CBD didn't block another CYP2D6 drug. Ondansetron is also generally well tolerated, so I wouldn't lose sleep here. Still, it's worth a quick chat with your pharmacist or doctor before combining them, especially with concentrated CBD products.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the gist: CBD can slow down one of the liver enzymes (called CYP1A2) that helps your body break down ondansetron. When that enzyme is working slower, ondansetron can hang around a bit longer and build up to higher levels than usual. Ondansetron is an active drug (not a prodrug that needs switching on), so more of it in your system simply means a stronger overall effect.

For most people the bigger thing to watch is ondansetron's known side effects, like headache, constipation, and feeling tired or lightheaded. One worth mentioning is a heart rhythm change called QT prolongation, which matters more at higher doses or if you take other drugs that affect the heart. The CBD doses studied were quite high. Before pairing the two, check with your pharmacist or doctor.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear ondansetron (Zofran). CBD can slow that enzyme down, so in theory ondansetron may stick around a little longer and build up in your blood. Ondansetron isn't a prodrug, so more drug means a stronger effect and a higher chance of its usual side effects, things like headache, constipation, or feeling dizzy.

One thing worth knowing about ondansetron is that at higher levels it can affect heart rhythm (a QT prolongation issue). That's not common, but it's a reason to be careful combining the two. Before pairing CBD with your Zofran, please check in with your pharmacist or doctor so they can weigh your dose and other meds.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
