Regorafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the short version: your liver uses an enzyme called CYP3A4 to help break down regorafenib (Stivarga). CBD can slow that same enzyme down. When the cleanup crew works slower, more regorafenib can hang around in your body than expected. This is an active cancer drug, not a prodrug, so higher levels mean a stronger effect and a higher chance of side effects.

With regorafenib, that could mean worse fatigue, hand-foot skin reactions (sore, blistered palms and soles), high blood pressure, diarrhea, mouth sores, or changes in liver tests. We have solid evidence CBD can raise levels of other CYP3A4 drugs, so please don't add CBD without first checking with your oncologist or pharmacist.

What Is Regorafenib Used For?

Brand name: Stivarga

Stivarga is approved to treat three types of advanced cancer in adults. It is used for metastatic colorectal cancer (cancer that has spread) after prior chemotherapy and targeted therapy; for locally advanced, unresectable, or metastatic gastrointestinal stromal tumor (GIST) after two previous treatments; and for hepatocellular carcinoma (liver cancer) after treatment with sorafenib.

All three uses are specific to Stivarga tablets taken by mouth. It is intended for patients whose cancer has progressed despite earlier lines of therapy.
