Here's the key thing about clarithromycin: it's a strong blocker of a liver enzyme called CYP3A4. That same enzyme helps break down THC, the main active ingredient in cannabis. So while you're on this antibiotic, your body may clear THC more slowly, letting it build up a bit higher than usual.

In plain terms, that means cannabis could hit you harder or last longer, with more chance of feeling dizzy, drowsy, foggy, or anxious, or having a faster heartbeat. I'll be honest though, this is a theoretical concern based on how the enzyme works, not something proven in studies. It's worth a quick chat with your pharmacist or doctor, especially if you notice you feel more impaired than normal.

Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist for clarithromycin (Biaxin). Your body clears this antibiotic partly through a liver enzyme called CYP3A4. Lab studies suggest cannabis, especially CBD-rich products, can slow that enzyme down a bit. In theory, that could let clarithromycin build up a little higher than usual, which might mean more of its typical side effects like stomach upset, nausea, diarrhea, or a strange taste in your mouth.

I want to keep this in perspective, though. This is based only on test-tube research, not real patient cases, so it's more of a 'possible' concern than a proven one. Clarithromycin is also usually a short course. Still, since clarithromycin itself can affect the heart's rhythm, it's worth a quick chat with your pharmacist or doctor before combining the two.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
