Here's the gist for this pairing: your asthma medicine Zileuton can mildly slow down a liver enzyme called CYP3A4. THC, the part of cannabis that gets you high, is partly cleared by that same enzyme. So in theory, taking the two together could let THC build up a bit more than usual, meaning a stronger or longer-lasting effect (more drowsiness, dizziness, a racier heartbeat, or feeling more impaired).

I want to be honest, though: this is only a theoretical concern based on how these compounds are processed, not something proven in real patients. Zileuton isn't a strong blocker, so any effect is likely modest. Cannabis affects everyone differently, so go slow, don't drive if you feel off, and check in with your pharmacist or doctor before combining them.

Theoretically, cannabis might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that the cannabis constituents delta-9-tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol moderately inhibit the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist with Zileuton (Zyflo) and cannabis. Your liver uses an enzyme called CYP3A4 to help clear Zileuton. Lab studies suggest cannabis (especially CBD and CBG) can gently slow that enzyme down. In theory, that could let a bit more Zileuton build up in your body than usual.

Zileuton isn't a prodrug, so more of it would mean a stronger drug effect and possibly more of its known side effects, like nausea, stomach upset, headache, or strain on the liver. About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
