Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Cilantro interacts with.

In vitro research suggests that extracts of cilantro can inhibit platelet aggregation. Theoretically, cilantro might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Voltaren, Cataflam, others), ibuprofen (Advil, Motrin, others), naproxen (Anaprox, Naprosyn, others), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, warfarin (Coumadin), and others.

Here's the thing to keep in mind: alteplase (Activase) is a powerful clot-busting medicine given in the hospital, usually as a one-time emergency treatment for a stroke, heart attack, or major clot. It already carries a real bleeding risk on its own. Lab studies hint that cilantro may slightly slow down platelets, the tiny cells that help form clots, so in theory it could add to that bleeding risk.

In real life, this is mostly a theoretical concern based on test-tube data, not human studies. Normal cilantro in your food is very unlikely to matter. Since alteplase is given in a closely monitored setting, your medical team is already watching for bleeding. If you take concentrated cilantro supplements, just mention it to your doctor or pharmacist.
