Cnidium and Diclofenac Sodium

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Diclofenac Sodium

Diclofenac Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Cnidium interacts with.

Laboratory research shows that osthol, a constituent of cnidium, inhibits blood clotting and the activity of platelets. Theoretically, cnidium might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Some anticoagulant or antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with diclofenac and cnidium. Diclofenac is an NSAID, and like other pain relievers in that family, it already has a mild tendency to thin the blood and irritate the stomach lining. Cnidium contains a compound called osthole, which in lab studies appears to slow platelets and clotting. Put them together and, in theory, you could nudge your bleeding risk a little higher.

I want to be honest about the strength here: this is based on test tube and animal research only, so it's more of a caution than a proven problem. Still, watch for signs like easy bruising, nosebleeds, dark or bloody stools, or a stomach ache that won't quit. Before adding a concentrated cnidium supplement, run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Diclofenac Sodium page

“Diclofenac (Voltaren), a daily mainstay for arthritis and joint pain, comes with a supplement list that calls for real vigilance. Of the 308 ingredients on file, 7 rate major, and roughly two hundred entries share a single theme: anything with blood-thinning activity adds to diclofenac's own bleeding risk, especially in the stomach.”
