Here's the gist with coconut water and alogliptin (Nesina): this is a very minor, mostly theoretical concern. The idea is that coconut water might nudge blood sugar down a little on its own. Since alogliptin already lowers blood sugar by boosting your body's own insulin response, stacking the two could, in theory, push your numbers slightly lower than expected.

But I want to be honest about the evidence here. This comes only from animal studies, not real-world people, so the actual risk is considered unlikely. A glass of coconut water as a refreshing drink is very unlikely to cause true low blood sugar. Just keep an eye out for shakiness, sweating, or dizziness, and if you drink it regularly, mention it to your pharmacist or doctor so they can keep tabs on your readings.
