Coconut Water and Ofloxacin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

Quinolone Antibiotics

Why this applies to Ofloxacin

Ofloxacin falls under Quinolone Antibiotics — the group Coconut Water interacts with.

Many quinolone antibiotics, such as ciprofloxacin and levofloxacin, may have reduced absorption when taken with calcium-containing products due to chelation. It is generally recommended to take quinolones at least 2 hours before or 6 hours after consuming oral products that contain calcium. The calcium content of coconut water varies by product but is generally between 100mg-300mg per 1,000mL. Consumption of a small amount of coconut water may be safe but it would be prudent to avoid drinking large amounts at the same time as a quinolone antibiotic. Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about ofloxacin and coconut water. Ofloxacin is a fluoroquinolone, and these antibiotics can grab onto minerals like calcium in your gut and form a clump your body can't absorb well. Coconut water carries some calcium (and a bit of magnesium), so chugging a big bottle right alongside your dose could lower how much antibiotic actually gets into your system. That matters because under-absorbing your antibiotic can mean your infection isn't fully treated.

The fix is simple: separate them. Take ofloxacin at least 2 hours before, or 6 hours after, drinking coconut water. A small sip likely won't hurt, but don't pair a large serving with your pill. When in doubt, your pharmacist can help you time it.

About Coconut Water

What is Coconut Water?

Coconut Water
  • Hydration
  • Rehydration after exercise
  • Electrolyte replacement
  • General wellness drink
  • Blood pressure support

Coconut water is a natural drink rich in potassium and other electrolytes that many people use for hydration, especially around exercise. For most healthy people it is a safe, tasty beverage, but it is not a proven medicine and should not replace medical electrolyte solutions for serious dehydration. People with kidney disease or those on potassium-affecting medicines should check with their pharmacist or doctor first.

About Ofloxacin

What Is Ofloxacin Used For?

Brand names: Oflox, Floxin, Floxin Injection, Floxin Otic

Ofloxacin is used in several different forms, each approved for specific infections. The otic (ear) solution treats outer ear infections (otitis externa) in patients 6 months and older, chronic middle ear infections with a ruptured eardrum (chronic suppurative otitis media) in patients 12 and older, and acute middle ear infections in children 1 year and older who have ear tubes. The ophthalmic (eye) solution — including the brand Ocuflox — treats bacterial conjunctivitis (pink eye) and bacterial corneal ulcers.

Ofloxacin oral tablets treat adults with mild to moderate bacterial infections including community-acquired pneumonia, skin and skin structure infections, gonorrhea, chlamydial infections of the urethra or cervix, mixed urethral/cervical infections, and acute pelvic inflammatory disease. The oral tablet is also used for acute bacterial exacerbations of chronic bronchitis and uncomplicated cystitis, but only when no other treatment options are available, due to serious risk concerns with this drug class.
