Phenobarbital, Phenytoin falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the thing to keep in mind with Dendrobium and your seizure medicines, phenobarbital and phenytoin. These drugs are prescribed specifically to raise the seizure threshold and keep seizures under control. Dendrobium contains a compound called dendrobine, which in lab reports has the opposite effect: it may actually promote convulsions. So in theory, adding Dendrobium could work against what your medication is trying to do and make a breakthrough seizure a bit more likely.

I want to be honest that this is a theoretical concern based on the plant's chemistry, not strong human studies, so don't panic if you've already taken some. Still, because seizure control is so important, I'd skip Dendrobium and check with your doctor or pharmacist first.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with anticonvulsant drugs might decrease drug effectiveness and increase the risk of seizure. Some anticonvulsant drugs include phenobarbital, primidone (Mysoline), valproic acid (Depakene), gabapentin (Neurontin), carbamazepine (Tegretol), phenytoin (Dilantin), and others.

Here's the simple version. You take phenobarbital and phenytoin (Dilantin with PB) to keep seizures from happening. Dendrobium contains a natural compound called dendrobine, and in lab and animal work it has actually been reported to trigger convulsions. So the worry isn't that it makes your medicine build up. The worry is that it could push in the opposite direction and work against what your seizure medicines are trying to do.

If that happened, the concern would be a breakthrough seizure. I'll be honest with you though: this is a theoretical caution based on how dendrobine behaves in studies, not on real patient reports. Since these are tricky drugs to keep balanced, I'd skip Dendrobium and check with your pharmacist or doctor first.
