Fentanyl citrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.

Here's the gist for fentanyl: your liver uses an enzyme called CYP3A4 to break it down and clear it out. Lab studies hint that devil's claw might slow that enzyme down. If that happened in your body, fentanyl could build up higher than expected, and that matters because fentanyl is a very potent opioid. More of it on board can mean stronger sedation, slowed or shallow breathing, low blood pressure, and confusion.

Now, the honest part: this is only test-tube research, not something seen in actual patients, so the real-world risk is uncertain. Still, fentanyl leaves no room for guesswork. Before combining the two, please check with your doctor or pharmacist so they can keep an eye on you.
