Alogliptin, Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Dhea interacts with.

Theoretically, DHEA might increase the levels of drugs metabolized by CYP3A4.
Some preliminary evidence shows that DHEA may inhibit CYP3A4; however, the clinical significance of this potential interaction is not known.

Here's the short version on taking DHEA with Oseni (which combines alogliptin and pioglitazone for type 2 diabetes). The pioglitazone part is partly broken down by a liver enzyme called CYP3A4. Some early lab hints suggest DHEA might slow that enzyme down a little, which could let pioglitazone build up slightly. In theory, that might nudge its effects a bit higher, things like fluid retention, swelling in the legs, weight gain, or a greater chance of low blood sugar if combined with other diabetes meds.

I want to be honest though: this is theoretical and based only on weak, anecdotal evidence, so we really don't know if it matters in real life. It's not a reason to panic. Just give your pharmacist or doctor a heads-up before adding DHEA so they can keep an eye on your blood sugar and any swelling.
